01632218 אוניברסיטת תל אביב הפקולטה לרפואה ע"ש סאקלר ביה"ס למקצועות הבריאות החוג לסיעוד ביטויי מיניות במצבי בריאות וחולי שעור, ש 2 "ס מועד הקורס: מרצה: שעות קבלה: סמסטר א', יום ג', בין השעות: 16:00-14:00 גב' ברכה גל דואר אלקטרוני: brachag@clalit.org.il לפי תיאום מראש. אינטלקטואלי והחברתי של הפרט ומהווה אחת נפשי, רציונל: מיניות מוגדרת כשילוב המרכיבים הפיזי, מפעילויות החיים היומיומיות בהם האדם נזקק לעזרה באם אינו מסוגל לעשותם בעצמו. קידום מענה לצרכים מיניים במצבי בריאות וחולי מהווה חלק מתכנית טיפול סיעודית הנדרשת עבור מטופלים על פי מודל ביופסיכוסוציאלי התואם לגישה ההוליסטית בסיעוד המתקדם. מטרות הקורס: לזהות ולגבש עמדות ואמונות לביטויי מיניות של עצמו וזולתו. 1) לפתח מודעות רגישות וסובלנות לדפוסי התנהגות מינית שונה של הזולת. 2) לבחון את מצבי הבריאות והחולי המשפיעים על מיניות האדם. 3) לאפשר פתיחת ערוץ תקשורת בין אחיות ומטופליהן בתכנים הקשורים במיניות. 4) להעלות את רמת המודעות לגבי מיתוסים ומסרים הקשורים במין. 5) להשוות את הגורמים המשפיעים על ביטויי מיניות בחברות שונות. 6) להבין את התפקידים המסורתיים המיניים 7) לדעת את שלבי הגדילה וההתפתחות המינית לפי רצף החיים. 8) לתפוס את חשיבות החולי, הנכות ודימוי הגוף כמשפיעים על ביטויי מיניות. 9) לזהות את המין כתקשורת. 10) לרכוש מיומנויות סיעודיות לשיפור הבריאות המינית של המטופלים. 11) שיטות הוראה: הרצאות, דיונים, פעילויות למידה בקבוצות. 1
נושאי השעורים: מיניות הגדרות פיזיולוגיה של המיניות במצבי בריאות וחולי מודלים להתערבויות בטיפול מיני דרישות הקורס: השתתפות בכל השיעורים, 1) השתתפות פעילה בתרגילי סמולציה 2) ביצוע כל המטלות והגשתם במועד. 3) דרישות מוקדמות: אין. ציון: תרגילים למידה פעילה בכיתה 50% הצגת מאמרים בכיתה 50% ע"פ הנחיות שיוגשו ע"י המרצה הערה: אי השתתפות בשיעורים ובמטלות כרוכה בהורדת 10% מהציון. ספרות חובה: הספרות תבחר בהתאם לנושא קליני הנבחר. ספרות מומלצת: הספרות תבחר בהתאם לנושא קליני הנבחר. 2
קווים מנחים להתערבות סיעודית בתחום המיניות הציגי את נושא המיניות באופן רגיל כחלק מטיפול ההוליסטי. הבטיחי זמן ופרטיות מרבית. שאלי האם המטופלת מעוניינת בנוכחות של בן הזוג. אל תמשיכי ללא קבלת הרשות של המטופלת. בררי האם המטופלת מבינה את נושא השיחה. תהי מודעת לכך שחלק מהמונחים עלולים להיתפס כפוגעים ברגשותיה של המטופלת. השתמשי בשפה שנוחה עבורך ועבור המטופלת. הציעי מידע כתוב או מילולי. אל תניחי הנחות. תהי מודעת לדעות הקדומות שלך בנוגע לאוננות, זהות מינית, משחקי מין וכד'. הכירי את המשאבים הקיימים עבור המטופלת ובני זוגן בנושא המיניות כמו: עמיתים, יועצים, מומחים, קבוצות תמיכה וכו'. הציעי תחליפים מעשיים בתחום המיני כמו: מגע, חיבוק, משחק מיני, ארוחה לאור הנרות, מקלחת משותפת וכד'. הכירי במגבלותיך. אפשרי למטופלת לשאול שאלות. הבטיחי למטופלת ובן זוגה זמן להיות לבד לאחר השיחה. עודדי, תמכי והתחשבי בדברים הבאים: תפקוד מיני של המטופלת לפני הופעת המחלה. השלב ההתפתחותי של המטופלת. מצב נפשי של המטופלת. מערכת תמיכה של המטופלת. קווים מנחים להתערבות סיעודית בתחום המיניות נשים לקראת ניתוח Hysterectomy בצעי הכרות עם בן הזוג. הסבירי את תפקידך במחלקה כולל מתן הדרכה לקראת חזרה לפעילות המינית. תני לגיטימציה סמויה וישירה לשאול שאלות בנושא המיניות: לגיטימציה סמויה: פזרי חורי הסברה בחדר ההמתנה ובחדרי הטיפול, ספרי הדרכה מינית, פוסטרים, תמונות וכד'. לגיטימציה ישירה: שאלי שאלות פתוחות המאפשרות לפתוח בשיחה על מיניות כמו: "רוב הנשים, אשר עוברות ניתוח להסרת הרחם, מעלות שאלות בנוגע למיניותן או תפקוד מיני לאחר הניתוח. איזה שאלות יש לך בנושא? Permission הסבירי על מבנה של אברי המין ותפקידם בחוויה המינית. הסבירי לאישה שאחרי הניתוח רוב הנשים ממשיכות לחוות אורגזמה. הסבירי, כי חלק חשוב בהשגת האורגזמה הנשית הינו קליטוריס והוא לא נפגע בניתוח. הסבירי, כי בתקופת ההחלמה לאחר הניתוח, כשאין אפשרות לקיים יחסי מין מלאים, ניתן להגיע לעוררות מינית ע"י מגע, ליטוף, חיבוק, נשיקות וכד'. הבהירי, כי העוררות המינית בתקופת ההחלמה איננה מהווה נזק היא מזרזת תהליכי ריפוי. Permission הסבירי לאישה ובן זוגה על שינויים אפשריים בתחום המיני, אשר עלולים להתרחש בעקבות המחלה והטיפול הכימותרפי. - תחושת העייפות עלולה להקטין את הרצון והיכולת לקיים יחסים, למרות שמבחינה רפואית אין הגבלה על קיום יחסי מין בזמן טיפול כימוטרפי. - חלק מהנשים עשויות לחוש תופעות של גיל המעבר כמו גלי חום, יובש בנרתיק וכד'. - הזכירי, כי אצל נשים צעירות יתחדש המחזור החודשי בדרך כלל לאחר סיום הטיפול. - הסבירי על פגיעה אפשרית בפוריות. - צייני, כי רוב התופעות שהוזכרו לעיל חולפות בדרך כלל לאחר סיום הטיפולים. Addressing patients sexual needs during sickness and health is a part of the nursing treatment plan required for patients according to the biopsychosocial model that corresponds with a holistic approach to advanced nursing. Goals of the course: 1) To identify and formulate attitudes and beliefs regarding the expression of sexuality for themselves and others. 2) To develop awareness, sensitivity and tolerance for the different sexual behavior patterns of others. 3) To examine the states of sickness and health that influence human sexuality. 4) To enable the opening of a channel of communication between nurses and their patients on subjects relating to sexuality. 5) To raise the level of awareness regarding myths and messages relating to sex. 6) To compare the factors that influences the expression of sexuality in different societies. 7) To understand the traditional sexual roles. 8) To know the stages of sexual development and growth along the human life span. 9) To understand the importance of illness, disability and body image as these impact expressions of sexuality. 10) To identify sex as a means of communication. 7
11) To acquire nursing skills to improve the sexual health of the patients.
